Inpatient Rehabilitation
Amputation - Rehab nurses and physicians will work with the patient on an individualized plan to manage swelling, pain and changes in sensation. Physical and occupational therapy will work with the interdisciplinary team to shape the residual limb and maximize the patient's independence with self care and mobility. The staff will also focus on education to help patients prepare for a safe transition home.
Arthritis - Our team is experienced in the management of patients with arthritis, including rheumatoid arthritis and osteoarthritis. The physicians are adept at managing medications and making adjustments to maximize rehab potential. Physical and occupational therapists will teach the patient to compensate for their limitations to promote independence and reduce pain during daily activities. The team will work with the patient and caregivers to ensure their home is set up for success and safety after discharge.
Brain Injury - The Northwest Transitions rehab team provides services for people with brain injury following accidents, surgeries or other neurological events. Our team is dedicated to treating the entire person. The nurses, physicians and therapists specialize in neurological rehabilitation. Speech therapy teaches the patient and caregivers strategies to manage changes in memory and attention. Physical and occupational therapy work towards improved strength and balance to support independence during daily tasks. Social workers and case managers will help identify community resources to support a safe transition home.
Cardiac Care - Our team specializes in caring for people with congestive heart failure, stents, heart attacks, coronary bypass surgery, pacemakers, valve replacements and other cardiac events. Our nurses and physicians will monitor cardiac labs and vital signs, adjusting treatment plans when indicated. The therapists will provide individual assessments to design a cardiac rehabilitation program specific to each person. The interdisciplinary team will teach patients and families how to manage these conditions optimally.
Diabetes - Our team of rehabilitation nurses and doctors are skilled in managing both new onset and pre-existing diabetes diagnoses. We will complete education for home management of blood sugar levels. We will consult dietitians for personalized education and diet training. Physicians and nurses will monitor labs throughout the patient’s stay, adjusting treatment if indicated.
Fractures and Joint Replacements - Our team specializes in providing care for patients with fractures, both those who have had surgery and those who have not. We also have experience managing patients after total and partial joint replacements for the upper and lower body. Our rehab nurses are trained to help with pain management post surgery to ensure patients can benefit and participate in therapy. The physical and occupational therapists will help patients progress to walking, dressing and completing home care tasks independently.
Multiple Trauma - Our team has experience assisting patients in their recovery following any type of accident or trauma that results in an inability to care for yourself. We can teach patients strategies to increase their ability move independently and perform self-care. Our nurses and physician will assist in pain management and will monitor for any complications. The nurses will care for any incisions or surgical sites to ensure they heal completely and reduce any risk of infection. Our social workers and case managers will identify community resources and equipment needed to support the patient and their family when they return home.
Neurological Disorders - We care for patients with neurological conditions including Parkinson’s disease, multiple sclerosis, myasthenia gravis, Guillain-Barre syndrome, ALS and other neurological disorders. Our team will assist in identifying the equipment and community resources necessary for a successful transition home. They will also work toward improving strength, balance and coordination. The physicians will adjust and initiate medications to optimally manage the condition. The team will complete caregiver education prior to discharge to ensure the right level of assistance is in place.
Pulmonary - Our team has experience managing patients with a history of new diagnosis of COPD, pneumonia, recent COVID and respiratory failure. Our nurses and physician will monitor oxygen levels and adjust treatment accordingly. Physical and occupational therapists will help improve endurance, teach breathing techniques and improve strength to support self care and mobility at home. Social work and case management will assist with setting up any needed equipment at home, including ongoing oxygen needs.
Spinal Cord Injury and Spine Surgeries - Our interdisciplinary team will work with the patient and their support system to maximize independence with daily activities and provide caregiver training to ensure the patient has the assistance they need to return home. Social workers and case managers will work to identify and obtain equipment as well as community resources to support a successful transition home. Physical and occupational therapists will work to improve strengthening, balance and endurance to maximize functional recovery.
Stroke - Our team has therapists with special training in stroke recovery. Our goal is to provide an individual treatment plan that promotes strength, balance and recovery of self care and mobility skills. Speech therapy provides specialized services in swallowing, speech, communication and cognitive disorders. Our nurses and physicians will monitor vitals and complete stroke prevention education. Physicians adjust medications as indicated to manage stroke risk factors. Social workers and case managers will identify equipment and support services to promote a successful transition home.
Other Complex Medical Conditions - Our team is skilled in managing complex medical conditions, including end-stage renal disease requiring dialysis, recent COVID hospitalizations, post ICU hospital care, sepsis, cardiac arrest, respiratory failure and multiple organ failure. Our physicians include medical internists who are skilled in managing a variety of medical conditions. Rehab nurses and physicians monitor labs and vitals, adjusting medications and interventions as needed to optimize recovery. The physical and occupational therapists will help the patient improve strength, balance, endurance, functional mobility and self care. The team will complete patient and caregiver training to support a safe discharge home. The social worker and case manager will help identify and obtain equipment and resources from the community.
